Furnish all labor and materials and perform all work required for the boiler replacement at Ramsey Middle School, 6409 Gellhaus Lane, Louisville, KY 40299. Bidders are required to submit the following with their bid: properly signed Proposal Form with completed unit prices and a listing of all sub-contractors; a listing of manufacturers must be submitted within 1 hour following the bid. Bid Security must be properly signed, witnessed, and executed. A Minority, Women, and Veteran Business Enterprise Sub-contracting Plan, including the Contractor's Good Faith Effort Summary Sheet, Affidavit, and Supplemental Sheet, must be properly completed, signed, and dated. All documents must be submitted as follows: the Bid Bond in one envelope, the Minority, Women, and Veteran Participation Documents in a second envelope, and the Form of Proposal in a third envelope. All three envelopes must be distinctly labeled for content and placed in a large single envelope labeled with the project name and submitting company. The contents of envelopes 1 and 2 must be complete for the bid to be considered. A Bid Guaranty in the form of a Certified/Cashier's Check or Bid Bond, in an amount not less than five percent (5%) of the bid, must accompany each submission and be made payable to Jefferson County Public Schools. The Bid Guaranty must be issued by a Surety Company rated "A-" or better by A.M. Best Company. Absence of this document will result in automatic rejection. Bids may not be withdrawn for 45 days after the deadline. The contractor's profile form, if not currently on file, is due prior to contract award. Nonconformance to requirements will be grounds for rejection of the bid. The owner reserves the right to accept, reject, or waive informalities as deemed in the owner's best interest and to reject any bid not satisfying the owner's qualification requirements.
