Question deadline 01/08/2026 at 03:00 PM Furnish all labor and materials and perform all work required for boiler replacement at Luhr Elementary School, 6900 Fegenbush Lane, Louisville, KY 40228. Submit sealed bids including a properly signed proposal form with completed unit prices and listing of all sub-contractors, a properly signed, witnessed, and executed bid security, and a completed Minority, Women, and Veteran Business Enterprise Sub-contracting Plan, Good Faith Effort Summary Sheet, Affidavit, and Supplemental Sheet. A listing of manufacturers must be submitted within one hour following the bid. Place the bid bond in one envelope, Minority, Women, and Veteran Participation Documents in a second envelope, and the Form of Proposal in a third envelope. Place all three envelopes in a large, single envelope labeled with the project name and submitting company. Non-conformance to these requirements will be grounds for rejection. Refer to specified sections for liquidated damages, substantial/final completion dates, and requirements for 100% performance and payment bonds.
