Birch Design Front Door Ramp, CR#: 2130060, is a Design ONLY project located at Birch IRA – 105 Hammond Road, Thiells, NY 10984. The contractor is to provide and supply a 1-year warranty on material and labor. Contractors must be registered before submitting a bid and prior to commencing any new work. Subcontractors are not allowed without written approval. When submitting a bid, bidders must provide their NYSDOL registration number and Certificate of Registration. Failure to provide these will result in disqualification of the bid. All quotes must be received by the designated deadline and must reflect the correct prevailing wages, using the provided wage schedule. Only the provided quote proposal form may be used—other forms will not be accepted. Email submission is preferred. A mandatory site visit is required for eligibility to submit a quote. Attendance must be confirmed in advance, and late arrivals are not permitted. In the event of a tie, the contract will be awarded to the earliest received bid. The awarded vendor must adhere to all timeframes, demonstrate two years of relevant experience upon request, and provide certificates of Liability Insurance and Worker’s Compensation Insurance, along with completed Lobbying Law Forms and signed Appendix A. All staff at the project site must sign an attendance sheet. The Hudson Valley DDSO reserves the right to reject any and all quotes.
