Description: Bridge replacement with approach roadway and related work. Project Location: : Washington Road over Coon Creek, 1/2 mile east of Clinton, IL. Proposals requiring prequalification of bidders will be issued only to those contractors who are qualified with the Department of Transportation. Prequalification forms may be secured from the Engineer of Construction, Administration Building, 2300 South Dirksen Parkway, Springfield, IL 62764 ILDOT: Bridge Replacement With Approach Roadway and Related Work The work included in this contract consists of: (1) The complete removal and replacement of the existing two-span structure, (2) Aggregate Base Course with an A-3 surface on the approach roadway, (3) A4 Riprap with filter fabric and (4) Seeding and other collateral work necessary to complete the improvement in accordance with the plans and as specified herein. This project is located 0.5 miles east of Clinton. Steel cost adjustments will be made to provide additional compensation to the Contractor, or a credit to the project owner, for fluctuations in steel prices when optioned by the Contractor. The bidder shall indicate on the attached form whether or not this special provision will be part of the contract and submit the completed form with his/her bid. Failure to submit the form or failure to indicate contract number, company name, and sign and date the form shall make this contract exempt of steel cost adjustments for all items of steel. Failure to indicate "Yes" for any item of work will make that item of steel exempt from steel cost adjustment.
