Repairs are required for Units 5 & 6 at the Boaz Recreation Center. The inducer motor on PKG-6 has failed and prevents proper draft and safe operation of the heating system. The gas valve on PKG-5 has also failed and is not regulating gas flow as needed for safe operation. The scope includes replacing the failed inducer motor on PKG-6, installing a new gas valve on PKG-5, providing all necessary parts, labor, wiring, adjustments, and testing each unit through a complete heating cycle to ensure safe and reliable operation. Bids must be submitted in a sealed and clearly marked envelope with all required forms signed and notarized. Bidders must provide a list of all equipment to be used, manufacturer’s brochures, a certificate of workers compensation and $2 million in liability insurance, possess a City of Boaz business license, and be a licensed HVAC contractor with a copy of the license included. The bid must contain a firm price, warranty information, and may not be amended or withdrawn after the scheduled opening time. The City reserves the right to accept or reject any and all bids.
