Roof replacement and/or roof recover at Northwestern Elementary School and Northwestern Middle School as indicated in the Construction Documents. Bids must be executed on Indiana State Board of Accounts Form 96, Revised 2013, and include completed Supplementary Bid Forms, Non-Collusion Affidavit, Bidder's Financial Statement, a Bid Bond or certified check in the amount of 10% of the bid price, and a signed Non-Discrimination Affidavit (or MBE-1). Successful bidders will be required to provide a Performance Bond and Labor and Materials Payment Bond for 100% of the contract amount. Costs for bonds and insurance must be included in the bid. Each bid envelope must be marked "Roofing Bid" for Northwestern Elementary School and/or Northwestern Middle School as appropriate, with the name of the Roofing Contractor and Project Name clearly indicated. Bids remain in effect for 60 days. Faxed or emailed bids are not accepted. The contract will be awarded to the best responsive and responsible qualified bidder(s). Work may commence upon Notice to Proceed and must be completed by the specified date. The Owner reserves the right to reject any and all bids, is not obligated to accept the lowest or any other bid, and may waive any bidding formalities.
