Contact Info Farrah Lim/707-645-2627 farrah.lim@cityofvallejo.net Scope of Services The work generally consists of but is not limited to: o Provide concrete inspection services for designated City-owned public sidewalks to include; location of offset, before photos, complete measurements of offset, and photos of repaired offset. This information shall be provided as a report to the City in a format that is compatible with the City's Geographic Information System (GIS) software (Esri Shapefile or Esri GeoDatabase). o List of grind or shave locations shall be determined by the City after report is provided by contractor. o Furnish all labor, materials and equipment necessary to complete tasks. o Contractor shall remove and dispose of existing temporary ramps at no extra charge. Adherence to all clean water regulations is required. o Contractor shall provide dust control by use of high efficiency vacuum attachment secured to the portion of the cutting or shaving machine. o Contractor shall collect, remove and dispose of all cutting or shaving residue at no additional cost. Adherence to all clean water regulations is required. o Contractor shall install traffic controls, as required, in compliance with the most recent adopted California Manual on Uniform Traffic Control Devices (CA MUTCD). o On-Call response time: Contractor shall mobilize within 48-hours, providing the necessary workforce and equipment, when requested by the City to inspect and repair by concrete cutting the offset or damaged sidewalk. If concrete cutting is not a feasible repair option, Contractor shall notify the city. The bid security for the three acceptable lowest Bidders shall be retained until the City Council has awarded a Contract to the successful Bidder. All other Bidders' bid security shall be returned. Pursuant to Section 22300 of the Public Contract Code, the Contractor may substitute a deposit of securities in lieu of the City of Vallejo withholding any monies to ensure Contractor's performance under the Contract, or alternatively, request that the City of Vallejo make payment of retentions earned directly to an escrow agent at the expense of Contractor. The provisions of Public Contract Code Section 22300 are incorporated herein by reference as though set forth in full and shall govern the substitution of securities and/or escrow account. The City of Vallejo reserves the right to reject any or all bids or portions thereof, to accept a bid or portion thereof; or to waive any minor irregularity. Bidders and Contractors shall be licensed in accordance with the provisions of Chapter 9, Division III, of the Business and Professions Code Section 7000 et seq., known as the Contractors State License Law. The license classification required for this project is A. Liquidated damages that may be assessed by City of Vallejo for late completion is $1,500 for each calendar day delay.
