Furnish and Install 4 - Stair Glides. 2 units with power folding fail and all units with extra wide seat and footrest. Furnish and Install 1 - Bruno Vertical Platform Lift. 36" x 38" upper right hand top gate with cold weather package. Use the existing outlet on wall behind the lower landing parking station. Call-stations on both landings and there is one on the arm of the stairlift. The work requested is a home modification for a participant in Binghamton, NY, identified as Re-Bid JQ 2-5-26. Bidders are required to thoroughly inspect the residence prior to bidding, as no additional work or funds will be authorized due to unawareness of existing conditions, and no changes to the contract amount can be made after it is issued. Bidders must return hard copies of the signed VR-340 Invitation for Bid form, Non-Collusive Bidding Certification, Executive Clause, and Offerer Disclosure of Prior Non-Responsibility Determinations. Submissions must include an actual cost breakdown for materials and labor, as well as warranties. Proof of Workers' Compensation Insurance and Disability Benefits Insurance is required, with only specified forms being accepted. All documents must be received in hard copy at the specified address; emailed or faxed copies will not be accepted, and late or incomplete packages may be disqualified. Site visits are available upon request, but no photos or videos of the site are provided.
