Such request shall be in writing, addressed to the Ownerís representative stipulated in the specification. In order to be given consideration, a written request must be received at least seven (7) business days prior to the date fixed for the opening of the bid. The work consists of sidewalk repairs at various locations in Union City. The scope includes removal of existing curb and sidewalk, construction and installation of new concrete sidewalk, concrete curb, and concrete driveways at various locations. Performance of the work must commence upon the Notice to Proceed and achieve substantial completion within 45 calendar days, subject to authorized adjustments. Proposals must be submitted on the standard forms as designated in the contract documents, enclosed in sealed envelopes bearing the name and address of the Bidder and the project name, and must be accompanied by a Statement of Consent of Surety from a surety company authorized to do business in New Jersey, along with either a Bid Bond or a Certified Check for not less than ten percent of the amount bid (not to exceed $20,000). The successful bidder must provide a performance bond and labor and material (payment) bond for the full amount of the project. One original proposal package and two complete copies of the bid package (clipped, not stapled) must be submitted within the same sealed envelope. The Owner or Engineer reserves the right to require a complete financial and experience statement from prospective bidders. Proposals will only be accepted from bidders who have properly qualified in accordance with contract requirements. The right is reserved to reject any or all bids or to waive any informalities that are not detrimental to the Owner. The successful bidder must comply with Affirmative Action requirements, the New Jersey Prevailing Wage Act, Anti-Kickback Regulations, Worker and Community Right-to-Know Act, and Anti-Drug/Alcohol plan. The bid must also be accompanied by a list of names and addresses of all stockholders owning 10% or more of the stock.
