This project includes the replacement of the existing single-ply membrane roof system at Mishawaka Library, located at 209 Lincolnway East, Mishawaka, Indiana 46544. Bids shall be properly and completely executed on the Bid Form provided and accompanied by a non-collusion affidavit, Form 96 of the State Board of Accounts, financial statement, bid bond or certified check, each for a value of not less than ten percent (10%) of the Bidder's total bid amount, including all alternatives bids items and other items as noted. The successful bidder will be required to furnish Contract Performance and Payment Bonds for 100% of their contract amount prior to execution of contracts. Bonds shall be maintained in accordance with IC 36-1-12-13.1 and 36-1-12-14 and be in full force and effect for a period of at least 12 months after the date of Final Completion of the Contract. The Contractor is required to enroll in and verify the work eligibility status of all its newly hired employees through the E-Verify program. Bid shall be submitted with one original and two duplicate documents in a sealed envelope with Bid submission clearly marked: SEALED BID ENCLOSED MISHAWAKA PENN HARRIS PUBLIC LIBRARY CAPITAL IMPROVEMENTS - 2024 GENERAL OBLIGATION BOND ROOF REPLACEMENT MISHAWAKA LIBRARY. The Mishawaka-Penn-Harris Public Library reserves the right to reject any and all bids or parts thereof, waive all informalities or irregularities in bidding and to resolicit quotations. The bid shall be awarded to the lowest responsible and responsive bidder. Contractors must submit written requests for information and clarifications to the Architect no later than seven (7) working days prior to Bid Date. All information, clarifications and interpretations will be made by a written addendum.
