Scope of work consists of lead service line replacement in Sections 3 & 4 of the Village of Summit and all applicable restoration as defined in the plans and specifications. Instructions to bidders: Sealed bids must be submitted on documents provided by the Village of Summit. Proposal forms are available for download only from QuestCDN via the Novotny Engineering website for a non-refundable charge. Proposal forms are non-transferable. Only proposals obtained from, and with the approval of, Novotny Engineering will be accepted at the bid opening. All proposals must be accompanied by a bank cashier's check, bank draft, certified check, or bid bond for not less than five percent (5%) of the total amount of the bid, or as provided in the applicable sections of the standard specifications. Bidders are required to undertake specified affirmative efforts regarding disadvantaged business enterprises. Bidders must comply with applicable regulations from the Illinois Environmental Protection Agency, Davis-Bacon Act, Employment of Illinois Workers on Public Works Act, Illinois Works Jobs Program Act, and federal Build America, Buy America Act requirements. Compliance with the President's Executive Order No. 11246, as amended, and requirements in 41 CFR 60-4 is also required.
