The successful bidder shall supply all labor, materials, and equipment required to complete the work as set forth in the general provisions of the contract, including General and Supplementary Conditions and Technical Specifications. The proposed work includes the decommissioning, abandoning, and grouting of an existing irrigation well; drilling of a new irrigation well; installing the casing and screen; connecting the new irrigation well to the existing irrigation system; and associated miscellaneous work at the Maiaroto Sports Complex. Instructions to bidders: - Proposals must be submitted in a sealed envelope, addressed to the Township Clerk, Township of Voorhees, and identified as a bid for "Maiaroto Well Rehabilitation." - All bids must include one paper copy and one electronic copy in the form of a USB or flash drive. - Prices must be submitted on the appropriate form, properly filled out and duly executed, stating the unit price bid required to fulfill these specifications. - Bid security in the form of a certified treasurer's or cashier's check or bid bond in the amount of 10% of the bid amount, not to exceed $20,000, must be included, along with a Consent Surety Statement, Certification, and Certificate of Authority issued by the New Jersey Department of Banking and Insurance. - Bidders must conform with the requirements of P.L. 1975, c. 127 "Affirmative Action," and file the non-collusion affidavit, consent surety, and Public Law 2012, c. 25 (Disclosure of Investments in Iran). - Bidders must submit a valid Business Registration Certificate with the bid, as well as valid Business Registration Certificates for any subcontractors. - New Jersey Prevailing Wage Rates shall be made part of the contract where applicable. - An acknowledgment of receipt of all Addenda and a disclosure statement naming all stockholders who own 10% or more of the company must be included. - All challenges to bid specifications must be presented in writing within 3 business days prior to the bid opening. *The list of planholders and/or interested bidders is not being tracked by the stakeholders involved with this project.
