Site work and paving for a mixed-use development in Mill Hall, Pennsylvania. Completed plans call for site work for a 7,300-square-foot educational facility; for paving for a 7,300-square-foot educational facility; and 7,300-square-foot educational facility.
The work involves patch paving at Central Mountain High School, Central Mountain Middle School, and Mill Hall Elementary School. Services to be provided include: saw cutting and removing damaged pavement in designated areas and replacing with new pavement; all patched areas must have a minimum of 3 inches of 19mm base binder and 1.5 inches of 9.5mm wear course with approximately 7,300 square feet of pavement to be replaced; sealing all seams with tar; providing all necessary labor, materials, and disposal; disposing of all waste in accordance with laws and regulations of the Commonwealth of PA; carrying appropriate insurance for all operations and providing proof of coverage; complying with Pennsylvania prevailing wages; and ensuring all employees onsite have required legal clearances (Act 34, Act 114, and Act 151). Contractors must also submit a Material Safety Data Sheet for products, company background and contact information, references from three school districts within the past three years, and a non-collusion affidavit. Bids must be clearly marked as "KCSD Patch Paving Sealed Bid." All proposals must be submitted on the attached Proposal Form along with a certificate of insurance.
