Wayne County Hospital intends to enter an AIA A133 – 2019 Standard Form of Agreement Between Owner and Construction Manager as Constructor, where the basis of payment is the Cost of the Work Plus a Fee with a Guaranteed Maximum Price, for the construction of hospital renovations and additions on the Wayne County Hospital Campus at 417 S East St, Corydon, IA 50060. The preliminary estimated construction budget is $4,500,000 - $5,000,000. The project scope includes approximately 4,275 square feet of new construction additions and sitework, consisting of a 1-story Southwest Addition including a Linear Accelerator Vault and associated site work. Qualification responses must be received by 2:00 pm on May 27, 2026, via email in electronic PDF format. Responses must be legibly prepared with a minimum 10-point font size and not exceed a forty (40) page limit, equivalent to twenty (20) double-sided pages, in 8.5” x 11” format. The submission file must be marked “RFQ for Wayne County Hospital – Firm Name” and addressed to Daren Relph, CEO. The Owner reserves the right to ask for additional written information to assist in evaluation, and expenses and pre-contract costs will not be paid by the Owner. Submissions will be evaluated and scored by a Project Evaluation Panel based on the technical RFQ and evaluation process as outlined in the document. The Owner will issue an RFP to each contractor who meets the qualifications selection criteria.
