The work consists of District HVAC Boiler Preventive Maintenance and Repair Services at multiple locations. Proposers must deliver according to specifications and for the prices quoted. Prices shall be quoted F.O.B. delivered and must remain firm for the contract period. No proposals shall be accepted from or contract awarded to any person or entity in arrears or default to the Board of Education of Warren Consolidated Schools. Proposals must be submitted in two copies, in a sealed envelope with the proposal title, due date, and time. Quotations must include all packing, crating charges, and surcharges. Proposals shall be accompanied by a sworn and notarized statement disclosing any familial relationship between the owner or employee of the proposer and any Board member or Superintendent. A sworn and notarized affidavit of compliance with the Iran Economic Sanctions Act of 2012 is required. Proposers must acknowledge receipt of any addenda issued. The supplier and Warren Consolidated Schools agree not to discriminate against any employee or applicant for employment on the basis of any legally protected category and must take affirmative action to ensure opportunity and fair treatment. The successful proposer must provide compensation insurance if labor is required. References must be provided upon request. Proposals will not be considered unless submitted on required forms. The Board reserves the right to reject any or all proposals, split awards, or accept the proposal best serving its interest. Proposers may withdraw their proposals between submittal and opening. Consultation with the Fiscal Services Department regarding conditions, materials, or previous quotations is allowed.
