Furnish all labor, materials, equipment, and supervision for the trenchless sewer rehabilitation at the Moffitt Beach Campground, in accordance with the project plans and specifications. Work includes but is not limited to trenchless rehabilitation of approximately 3,600 linear feet of a gravity sewer collection system, complete rehabilitation of two sanitary sewer manholes, all point repairs to support the trenchless sewer rehabilitation, pumping septic tanks, and associated sitework and restoration. Each proposal must be accompanied by a deposit or a Bid Bond in the amount of 5% of the bid. The Apparent Low Bidder will be required to submit a completed New York State Vendor Responsibility Questionnaire for For-Profit Construction (CCA-2) and M/WBE-EEO and SDVOB Utilization Plans within 24 hours and within five days, respectively, of being notified as the Apparent Low Bidder. Proposals will be accepted only from bidders who obtain the biddable Contract Documents directly from the Department and must be submitted on a printed paper version of the official Proposal Form and enclosed in a sealed envelope. Subcontractors will be required to complete the New York State Vendor Responsibility Questionnaire for For-Profit Construction (CCA-2). There are maximum subcontracting limits as specified in the Supplemental Instructions to Bidders. All contractors' employees shall be certified as having completed an OSHA 10 safety training course. Successful Bidders will be required to furnish a Performance Surety Bond and a Labor and Materials Surety Bond, each for 100% of the amount of the contract, and provide Policies of Insurance as set forth in the Contract Documents.
