The work consists of removal of gravel and rip-rap, installation of new shot rock, rock base, concrete encasement, rip-rap, and other incidental items and work described or reasonably inferable from the contract documents, for the complete construction of the E. Plattin Street Pipe Encasement Improvements project. Bidders are instructed to: - Use the bid forms provided, completing all blanks by typewriter or in ink. - Submit the Non-Collusion Affidavit and 5% Bid Security with the bid, as well as a designation of the work to be performed by the bidder, the proprietary names and suppliers of major materials and equipment, and a Subcontractor Approval Form for all proposed subcontractors. - Enclose all bid materials in a sealed envelope labeled "SEALED BID ENCLOSED" with the project and bidder's information on the face, and deliver to the location specified before the deadline. - Understand that late bids will be returned unopened, and no modifications, withdrawals, or cancellations of bids are permitted within sixty days after the bid deadline. - Recognize the owner's right to reject any or all bids, to investigate bidder qualifications, and to award to the lowest responsive, responsible bidder. - The successful bidder must provide a performance bond and labor and material payment bond, each in an amount equal to one hundred percent of the contract sum, before execution of the agreement. - Comply with all federal and state wage requirements and employment verification, including providing an OSHA construction safety program for on-site employees. - Understand the project requires faithful and full performance as described in the contract documents or reasonably inferable therefrom.
