Stony Brook University Hospital proposes to purchase Exterior Signage Supply and Install and related items. Any vendor who believes they can provide this product or an equivalent is invited to respond. If you plan to provide other than the referenced brand name cited in the purchase description, you must include the alternate brand name, catalog/part number, and the characteristics of the alternate item that are equivalent to the referenced brand name. Provide all documents clearly describing the alternate item(s) so they can be evaluated. Failure to submit the requested alternate substitute item(s) information before the closing date will eliminate the vendor from further consideration. Items otherwise equal may not satisfy the requirement due to differences in material composition, size, environmental application, etc. If offering equivalent and compatible equipment, the vendor must provide, at no cost if asked, supporting documentation, software (as needed), transportation, set-up, removal, and equipment for examination, demonstration, and trial. The successful contractor is required to submit valid and current NYS Workman’s Compensation and Disability Forms and complete documents related to NYS Finance Law §139 j&k, and Public Officers Law. Vendors are recommended to file the required Vendor Responsibility Questionnaire online via the New York State VendRep System.
