Provide all labor, equipment, material, transportation, supervision, services and items necessary for the removal of current flooring, testing of moisture in substrate or sub-floor, determination of stability and life span of underlayment and raised floor structure, and installation of new flooring in accordance with manufacturers and current industry standard requirements. Bidders must submit a complete quote consisting of three parts: a signed Standard Form 1449 with total firm-fixed-price bid, a detailed price breakdown for major tasks (removal of existing flooring, surface preparation, installation of new flooring, site cleanup, and labor hours & wages), and a technical proposal not exceeding two pages that includes a step-by-step technical approach, a list of major materials and equipment (including technician and firm certification, and test instrument calibration certificates), and a high-level project schedule starting from Notice to Award and showing completion within the required period of performance. Do not submit extraneous documents, promotional materials, company history, or generic cover letters. Quotes must only contain the specific information requested.
