The work, in general, will include the following approximate quantities and items, including all incidental work: Base Bid includes 50 LF 24" sanitary sewer, 84.04 VF 48" standard corrosive resistant manholes, 15.55 VF 48" drop corrosive resistant manholes, 47.34 VF 60" standard corrosive resistant manholes, 17.20 VF 60" drop corrosive resistant manhole with vent, 4 EA ditch checks, and 1 LS traffic control. Alternate Bid includes 71.88 VF 48" standard manhole rehabilitation (polymer manhole insert), 15.55 VF 48" drop manhole rehabilitation (polymer manhole insert), 47.34 VF 60" standard manhole rehabilitation (polymer manhole insert), 17.20 VF 60" drop manhole rehabilitation (polymer manhole insert), 4 EA ditch checks, and 1 LS traffic control. All Contractors shall comply with the "Contractor Qualification Ordinance of the Village of Pleasant Prairie" requiring pre-qualification of Contractors prior to performing work on this Contract. All Contractor pre-qualifications must be complete and submitted 5 days prior to bid opening date time. Approved pre-qualifications will be posted prior to bid opening. All Subcontractors utilized for this Contract must be identified in the bid documents under "LIST OF SUBCONTRACTORS AND SUPPLIERS" and all sub-contractors must be pre-qualified prior to award of this Contract. The Bidder (Prime Contractor) shall perform, with its own organization, work amounting to not less than thirty percent (30%) of the total original Contract Price. Work performed by subcontractors, including subcontractors affiliated with the Prime Contractor, shall not be considered work performed by the Prime Contractor's own forces. At the Contractor's option, the following specialty items of work may be excluded from the total Contract value used in calculating the minimum self-performance requirement: contractor staking, electrical work, landscaping and erosion control work, traffic control work, signing work, and pavement marking work. The Prime Contractor must be prequalified in applicable work categories sufficient to perform the required minimum thirty percent (30%) of the Contract with its own forces. Prior to award, the Bidder shall, upon request by the Owner, provide a list of bid items proposed to be self-performed in order to evaluate compliance with this requirement. Failure to comply with this provision may constitute grounds for rejection of the bid or termination of the Contract.
