The work to be performed under this contract shall consist of milling, detectable warnings on curb ramps, resurfacing, pavement markings and signing as set forth in the plans and estimates. Project length is approximately 3,954 feet (0.75 miles). The Disadvantaged Business Enterprise goal for this project shall be 6%. Prior to bid opening, all bidders are required to submit the LPA DBE Utilization Form which details their DBE Utilization Plan. Prequalification - Bidders must apply for pre-qualification with the Ohio Department of Transportation (ODOT) Office of Contracts, Contractor Qualifications Section, at least 30 days before the date is set to open bids. Only those contractors which are then currently pre-qualified by ODOT will be eligible to submit bids for the construction of projects addressed by this policy. Certificate of Compliance with Affirmative Action Programs - No contract shall be entered into unless the bidder possesses a valid Certificate of Compliance with Affirmative Action Programs, issued by the State EEO Coordinator, Certification Section, 77 South High Street, 24th Floor, Columbus, Ohio 43215, dated no earlier than 180 days prior to the date fixed for the opening of bids. Each bidder is required to furnish with its proposal, a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid security furnished in Bond form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. Each Proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must be prequalified by the State of Ohio and submit evidence of its experiences on projects of similar size and complexity. The owner intends and requires that this project be substantially completed no later than 60 days after Notice to Proceed. If the bid is accepted, the successful bidder shall enter into a contract and the Proposal Guaranty and Performance/Payment Bond shall provide the conditions in said bond, and as listed below: (A) Contract Performance Bond in an amount equal to 100% of the estimated cost of the work conditioned, among other things that the Contractor will perform the work upon the terms proposed, within the time prescribed, in accordance with the plans and specifications. The Ohio Department of Transportation shall be a co-obligee on the bond. (B) A separate Payment Bond in an amount equal to 100% of the estimated cost of the work conditioned for the payment by the Contractor and all Sub-contractors for labor performed and materials furnished in connection with the project involved. City of Girard reserves the right to reject any or all bids and waive any informalities in bidding. Bids of Corporations not chartered in Ohio must be accompanied by proper certification that such corporation is authorized to do business in Ohio. Bidders must comply with the prevailing wage rates on Public Improvements in Trumbull County and City of Girard, Ohio as determined by the Wage and Hour Division of the U.S. Department of Labor. The successful low bidder will be required to comply with all Contract Requirements for Equal Employment Opportunities. Domestic steel requirements as specified in Section 153.011 of the Revised Code apply to this project. Copies of Section 153.011 of the Revised Code can be obtained from any of the offices of the Department of Administrative Services. The Contractor and all subcontractors for this project must be enrolled in the Ohio Bureau of Workers Compensation Drug-Free Workplace Program or in a similar program approved by the Bureau of Workers Compensation and in good standing with the Bureau of Workers Compensation. Any Contractor that does not meet this requirement will have their bid deemed non responsive and cannot be awarded the contract. Each bidder must insure that all employees and applicants for employment are not discriminated against because of race, color, religion, sex, national origin, sexual orientation, handicap, age, Vietnam-era veteran status or disability. No bids may be withdrawn after the scheduled closing time for the receipt of bids for at least sixty (60) days.
