All bidders must execute and file in the bid proposal An "Affidavit of Organization and Authority" and a "Bidder's Proof of Competency" questionnaire, which forms are included in the contract document. The questionnaire shall be kept on file and be valid for the calendar year, subject to the Contractor's status through the year. Additional blank forms of this questionnaire can be obtained from the Consulting Engineer's office. Prior filing of such form with the School District of Beloit Turner or Consulting Engineer for the current calendar year shall satisfy this requirement. The DBE Goal for this project is discretionary. Federal Wage rates are required for this project and are included in the specifications. The contractor and its subcontractors will be required to provide certified payrolls. The Contractor and its subcontractors are required to follow Affirmative Action as noted in FHWA form 1273 under section 2.1.b and is included in the specifications. To be considered a Prime Contractor, the contractor must perform at least 30 percent of the original contract amount with the contractor's own organization. The contractor's own organization is defined as workers the contractor employs and pays directly as well as equipment the contractor owns or rents, either with or without operators. No bid can be received or considered unless accompanied by a Bid Bond or a Certified Check equal to ten percent (10%) of the bid, payable to the School District of Beloit Turner, as a guarantee that, if the bid is accepted, the bidder will execute and file the proper contract and 100% Performance and Labor and Material Payment Bonds within ten (10) days from the date the lowest responsible bidder's bid is accepted. In the event the bidder fails to file such contract and bond within the time set by the School District of Beloit Turner, the check or bond shall be forfeited to the School District of Beloit Turner as liquidated damages. The School District of Beloit Turner reserves the right to reject any or all bids and to waive or not to waive any informalities in the bidding. After bids are opened, no bid can be withdrawn for a period of ninety (90) days thereafter.
