The County of Santa Clara (the "County") is requesting bids from vendors to provide Certification, Testing, and Maintenance for Underground and Above Ground Fuel Storage Tanks on an annual basis for the County of Santa Clara's Health System ("CSCHS") Bidder(s) must comply with the County's Vaccination Requirements and Health Orders and must submit the required documents during the course of the solicitation and award process, for: a) non-mandatory onsite conferences, site visits, or product demonstrations or any other event held at County premises, should Bidder(s) choose to participate. b) the selected Contractor, upon notification of an intent to award, if applicable and per County's sole determination. Please refer to the "County of Santa Clara COVID-19 Requirements and Contractor Certification of Compliance" form as attached in Periscope Holdings. Bidders must arrive at the meeting area between 10:00 AM - 10:15 AM. Bidders wanting to participate in the site visit will be required to submit a "Contractor Certification of Compliance with COVID-19 Vaccine Requirements" Form for each attendee. Please reference the form as attached in this ITB. Please email all forms to julius.yang@prc.sccgov.org by May 3, 2022 at 3 pm PST. Bidder must also provide proof of vaccination (photo is acceptable). The County's intent is to award a three (3) year contract with two (2) one-year options to extend the term which may be exercised in the County's sole and absolute discretion. Pricing shall remain firm and fixed for the first year term of the agreement. County and Contractor reserve the right to consider price adjustments not to exceed an increase of 3% per year.
