Washington Drive Sports Complex Phase I - Tennis Courts. All bids must be accompanied by a bid security equal to five percent (5%) of the sum of the Base Bid, and must be in the form of a certified check, cashier's check of Bid Bond written by a surety or insurance company complying with R.S. 38.2218 C. In accordance with 38.2212.(A)(l)(b)(ii)(bb) and 38.2227, the apparent low bidder will submit the CONTRACTOR'S AFFIDAVIT (AFF-1), CONTRACTOR'S ATTESTION (AC 1-2) and ATTESTATION CLAUSE REGARDING VERIFICATION OF EMPLOYEES (AV-1) within ten (10) days of the bid opening or they will be declared non-responsive. Any person requiring special accommodation shall notify the City Clerk of the type(s) of accommodations required not less than seven (7) days before the bid opening.
