The location of the work is the intersection of Main Street, Willard Peak Road, and Fish Hatchery Road in Mantua, Utah. The scope of work includes reconstructing the asphalt pavement and re-striping of the intersection of Main Street, Willard Peak Road, and Fish Hatchery Road to eliminate an irregular and potentially hazardous traffic geometry. Pavement will be re-configured along with a new striping plan to help line up traffic entering the intersection at perpendicular angles. Work includes: Removal of 18,525 sq. ft. existing asphalt, fine grading of existing base course and supplementing base course gravel leveling course, excavate a drainage swale on the west of the intersection, 17,425 square feet of 4" thick bituminous concrete mix, adjusting 2 manhole covers to grade, adjusting 1 water valve box cover to finish grade, and paint striping. The email with the attached required bid documents, specified below, will be opened and witnessed by at least one representative from Hansen and Associates and one representative from the Town of Mantua after the submitted email time stamp is verified to be on or before the due time and date. Bidders must have returned a pre-requisite Bidder Registration form in order for the bid to be considered by the Owner. Any bids received from Contractors who did not previously submit a Bidder Registration form will not be accepted. Scanned or digital copies of following documents are required for electronic bid submittal: a. Bid Form completed by bidder. b. Bid Schedule prices fully filled in by bidder. c. A scanned or photocopy of an executed Bid Bond specific for the project from bidder's bond company; or, d. A scanned copy of a certified cashier's check for 5% of bidders total bid price. Bid Security may be in the form a Bid Bond or cashier's check and will be returned to each unsuccessful Bidder after tabulation and award of the Construction Contracts. CONTRACTORS are recommended to visit and review the site on their own before submitting a bid and shall be responsible for becoming familiar with the conditions at the site independent of any pre-bid conferences at the site. Questions shall be submitted electronically to the ENGINEER at chrisw@haies.net and responded to electronically to all CONTRACTORS who have registered electronically with the ENGINEER. Bids for each Construction Contract shall be on a unit price basis per each bid item. The work will be substantially completed for the Construction Contract by October 15, 2023, unless justifiably extended by Change Order. Bids shall remain valid for 45 days after the day of Bid opening. Bidders who withdraw their bid after Bid opening, but before expiration of said period, shall forfeit their bid security if Notice of Intent to Award to the successful Bidder is made by OWNER.
