The purpose of this ITQ is to establish fixed pricing for decontamination, disinfection, and disposal of bio-hazardous waste services for the City of Miami Beach Police in accordance with the specifications contained in this document. The successful bidder shall furnish clean up, disinfection, decontamination, and disposal services of potentially infectious bloodborne pathogen materials, such as human body fluids, spills, and materials on a 24-hour, 7-day-a-week basis. The quote form (section 8) shall be completed in its entirety. All corrections on the quote form shall be initialed. Failure to submit the most recent quote form (either included in the original itq or released via an addendum) may result in bid being deemed non-responsive and not being further considered.
