The Department of Health (DOH), Hawaii State Hospital (HSH), is interested in contracting the labor, equipment, tools, and materials to perform all operations in connection with the Repair of the Fan Blower Assemblies in 3 units on the grounds of Hawaii State Hospital Repair Work a) Remove & dispose of equipment from York XTI AHU for buildings: F, H & I. b) Deliver & install one new fan assembly with new motor & any other parts necessary to return the air handler to normal service in each building. c) Installation work must be scheduled with Plant Ops. d) Adjust fan belts & align sheaves & any other work required for the new fan wheel assembly & motor to run properly. e) Start up & test to insure proper functioning. f) At completion of the work, Contractor will provide a written warranty that the completed project is free from any workmanship or material defects for a period of 1 year starting from the date of the works formal acceptance. The warranty shall state that, in case of any workmanship defects that will occur within the 1 year, the Contractor shall implement the necessary corrective work without charging the government for the costs of materials, labor & equipment use.
