Referencing the Work Plan and attached information for this project, we hereby submit our quote in the amount stated
in the blank areas of this quote form. Also the undersigned, having become thoroughly familiar with the terms and
conditions of the contract documents and with local conditions affecting performance and cost of the work at the place
where the work is to be performed, and having fully inspected the site in all particulars, hereby proposes and agrees
to fully perform the work within the specified time stated and in strict accordance with the Contract Documents.
Lump Sum for removal and disposal of all asbestos-containing materials specified in Section 2.1 of the Work Plan
provided. Please provide base bid pricing for each item as requested below in this Abatement Bid Form, and include
