Summary Park Backflow Testing and Repairs Background About the City The City of Lake Forest is located in southern Orange County, California, in the area commonly referred to as the Saddleback Valley, with a population of approximately 85,000. It is the County's thirty-first city and the second largest city within the Saddleback Valley, encompassing 16.6 square miles. The City of Lake Forest's fiscal year begins on July 1 and ends on June 30. The City was incorporated in 1991, operates under the Council/Manager form of government and is considered a contract city. The five members of the City Council are elected by district. They serve staggered four-year terms, with the Mayor being selected annually from among the Council Members. The Council meets on the first and third Tuesdays of each month. City Hall is located at 100 Civic Center Drive, Lake Forest, California 92630. Park Backflow Testing and Repairs Contractor shall conduct annual tests of up to 140 backflow prevention assemblies in Lake Forest, CA. Assemblies range in size from 3/4 " to 6". The City shall protect the public water supply from contamination by implementation of a cross-connection program. Such protection is enforced through the California Code of Regulations Title 17 Section 7584, which requires the City to comply with all applicable state and federal laws required by the Safe Drinking Water Act of 1974, as they are now constituted, or as they my hereafter be amended or recodify and implemented through the City's "Cross Connection Control Program". The Contractor must have a current calibrated testing gauge. The Contractor must ensure that the technician be clearly identifiable and carry proof of I.D. and technician certification as defined in the California Department of Public Health Drinking Water Title II. The Contractor must be able to present a proof of current Orange County Backflow Assembly Certification and proof of current test kit calibration from a certified Lab prior to being awarded a contract. The Contractor shall provide all necessary personnel, instruments, equipment, and materials to perform described services. The technician is responsible for all safety measures. Some of the assemblies may be located in the center median of busy streets and proper precautions should be taken. All technicians should be familiar with and practice all State and Federal OSHA codes and requirements. All vehicles must be equipped with beacons and/or strobes. Prior to award of contract contractor must provide at least three (3) references, past and present, using the same service being proposed and indicate the scope of work, and the name, email address, and telephone number of the client contact. Also, contract will be required to provide a complete list of other municipalities in California utilizing contractor services over the past five (5) years. Test Reports: Test reports are legal records and must therefore be complete and accurate. A complete and accurate test report is proof that a test was performed and the assembly will prevent backflow and cross connection contamination of the water supply. The Contractor will provide a test report form to the City for approval prior to usage. All final inspection reports will be sent directly to the corresponding water purveyor, either Irvine Ranch Water District, El Toro Water District, or Trabuco Water District, via the Contractor. All reports will be submitted prior to the deadline set forth by the water purveyor. If an assembly fails the initial test, the Technician must determine if it a minor repair or a major repair. A minor repair would be a simple cleaning or installation of a rubber kit. If an assembly is cracked or a check valve or relief valve needs to be replaced, the Technician is to notify the City to obtain authorization for the repair prior to commencement of work. Minor Repairs: Minor repairs that can be performed at the time of test and cost less than $100 are to be performed along with the retest and included in the testing invoice. Examples of this type of repair are cleaning surfaces and rubber repair kits and test cocks. The contractor shall request authorization for any such repair prior to the commencement of work. Major Repairs: All major repairs such as cracked assemblies or replacement of check valves will be documented and approved by the City prior to repairs being performed. Contractor's Responsibilities: Conduct testing Monday-Friday during the hours of 8:00 AM to 5:00 PM (excluding City-observed holidays) Prior to beginning of each testing day Contractor shall provide notification of planned testing area(s) as well as start and end times for that day. Such notification shall be submitted via email to the project manager or designee. Acquaint themselves with the existing City's master list of backflow assemblies Furnish all labor, materials, tools, equipment, supplies, facilities, vehicles, and supervision necessary to provide annual testing of backflow prevention assemblies Provide City accurate and complete individual original hardcopy of test report for each tested assembly when requested by City designee Provide City with annual test kit calibration documentation for all test kits used prior to annual expiration date Responsible through the life of the Contract to provide and keep current their test equipment, permits and licenses, insurance and certifications required by the City of Lake Forest to completed the work outlined in this bid Question Submission Deadline:January 16, 2024, 5:00pm Anticipated Contractor Selection Date:February 1, 2024
