American Fork City is soliciting proposals from qualified contractors to provide roadway striping and pavement message services throughout American Fork City for 3 years. Scope of Work and Tasks to Be Completed and Included Within Proposal Contract goals: o Minimize impact to residents, business, those traveling. o Provide a quality striping product on the city streets. o Be available when needed. o Work with city staff to meet MUTCD requirements. Successful contractor team is expected to fulfill the following: o Consist of individuals with experience in delivering similar types of projects. o Provide a project manager with project delivery experience with managing similar types of projects. o Repaint striping and messages to match the existing in the areas indicated. o City staff will provide exhibits and/or mark out areas that are to be painted different from the existing paint layout or if the paint did not exist prior. Provide a bid table using the following Estimated quantities for the 3 years. o Attachment D bid table o Include how small-scale projects will be handled that have a 2-week timeline. Include type of equipment that will be used on this project. Paving service including highway paving and road paving and infrastructure building services and parking lot and surfacing services Public safety and control equipment and supplies and accessories including traffic control (driver feedback signs and traffic signals) and water safety and rescue equipment and accessories and snow and ice melter and snow removal equipment and road salt Description: The purpose of the project is to provide roadway striping and pavement message services within American Fork City. Each year the public works department will focus on a part of town to maintain or update the existing striping, and the contractor will be able to perform this work on their schedule as long as it gets done within 2 months after the request. There will be occasions where striping modifications will be requested on a smaller scale as needed and the contractor will need to perform these tasks within 2 weeks of the request. These requests may fall under the small-scale mobilization costs. For small scale requests that are under $10,000 for long line striping and $5,000 for road messages the small-scale mobilization costs may be applied. The City Council Acceptance of Award will be issued on Tuesday April 23, 2024, and anticipated Notice to Proceed by April 28, 2024 All official notifications, addenda, and other Bidding Documents will be offered only through online. Neither Owner nor Engineer will be responsible for Bidding Documents, including addenda, if any, obtained online. American Fork City reserves the right to modify the following schedule at their discretion: Final day to submit questions April 3, 2024, before 2:00 pm. Proposers are encouraged to submit their questions ahead of time via electronically. If the RFP needs to be modified or clarified, a written addendum will be issued. Questions may be submitted through electronically. All the Q and A will be posted electronically. City issues final answers to questions April 9, 2024 City Council acceptance of award April 23, 2024 Anticipated Start of Services by April 28, 2024 Only those Proposers who attend the mandatory pre-proposal conference will be considered for award. Proposals shall include the following: Cover page Content (2-page max) Appendices (5-page max) RFP agreement PROPOSAL FORMAT All proposals shall be formatted according to the following specifications: Content and Page Limit: o Cover Letter - 1 Page; include the company, contact name, address, phone number and email address on the cover letter. o Proposals shall include information to meet the criteria used for evaluation. Proposals are limited to a maximum of two (2) pages single sided 8.5" x 11" sized page. One-inch margins and font size will be 11- point font minimum - Times New Roman or Arial. The cover letter does not count as one of the 2 pages. o Appendices may include the following (does not count toward the 2-page proposal total): Resumes - Include resumes of significant team members, limited to one page each. Provide a submittal showing the type of paint and application information. o The Proposed Pricing/Cost/Fees page. This may be 11x17 if needed for readability and does not count as one of the 2 pages. Agreement: Proposers for this RFP must be willing to enter into an agreement with American Fork City. A sample of the agreement is attached. Contractor further agrees to pay as liquidated damages, the sum of $100.00 for each day after the required completion date per work order. Proposer Qualifications: Proposer must provide evidence that they are a licensed, bonded, and insured contractor in the State of Utah. Bidder must have proof of Worker's Compensation Insurance with Utah statutory limits and carry Commercial General Liability Insurance with a combined single limit of $1,000,000 each occurrence for bodily injury and property damage. Upon award of the contract resulting from this Proposal, the successful party must submit a Performance Bond in the amount of 100 percent of the contract price, with a corporate surety approved by the U.S. Department of the Treasury (Circular 570, latest edition).
