The Capital Area Transportation Authority ("CATA') in Lansing, Michigan, is requesting pricing for OVERHEAD DOOR PREVENTATIVE MAINTENANCE AND SERVICE in accordance with the Scope of Work as described in Exhibit A. The term of service will be three (3) years. Service is requested at the following locations 1. CATA Administration Building, 4615 Tranter Street, Lansing, MI 48910 2. Future CATA Locations which could include buildings in Ingham, Eaton, and Clinton County. Contractor, at the direction of CATA's Facilities Manager, shall provide overhead door maintenance and service Preventative Maintenance Provide all services necessary for the routine preventative maintenance and repairs of overhead doors as outlined below: 1. Ten (10) exterior overhead, Wayne Dalton 2. Eleven (11) exterior overhead doors, Overhead Door 3. One (1) interior overhead doors, Overhead Door 4. Three (3) Overhead Door roll up fire doors 5. Two (2) Wayne Dalton roll up fire doors Overhear door preventive maintenance will be done once a year for all doors; this service will be done between May 1st and June 30th. See Attachment 1, "Facility Overhead Door P.M. Sheet". Repair Services Upon completion of regular planned preventative maintenance, if there is a need for repairs beyond the maintenance it will be brought to the attention of CATA's Project Manager. Contractor will provide CATA with a firm price for the indicated repairs. Sufficient details, including itemized costs and repair time durations, are required. Repairs will be performed upon receipt of CATA's written authorization to proceed. BID BOND. N/A PERFORMANCE & PAYMENT BONDS. N/A The Contractor selected must be a licensed company, and have valid business, automobile, and workers compensation insurance in effect.
