Contractor shall provide all necessary materials, supplies, tools, equipment, transportation, licenses, permits, prevailing wages, insurance, bonds, and personnel to: provide the DSH with shade structure installation services. he services shall be performed for the Department of State Hospitals at the following location(s): DSH - Coalinga, 24511 W. Jayne Ave., Coalinga, CA 93210 The term of any agreement resulting from this IFB is anticipated to be December 1, 2024 through November 30, 2026. Question Deadline 10/22/2024 at 12:00 PM PT Bidders shall notify the DSH immediately via e-mail at brandon.chastain@dsh.ca.gov and sherry.clark@dsh.ca.gov. DSH Response to Questions 10/25/2024 4:00PM. Bidder must provide a copy of current Contractors State License, California Contractors State License Board (CSLB), with an active B- General Building Contracts License issued by the California Department of Consumer Affairs, Contractors State License Board. Proof of current licensing must be submitted with bid packages. If not licensed for all disciplines required to complete the service, the Prime Bidder must utilize applicable subcontracting, and must confirm licensing is current and active throughout the duration of the service. The DSH reserves the right to reject all bids.
