This item shall consist of performing Roadway Excavation and Embankment, including channels, according to Item 203 of the Construction and Material Specifications of the Ohio Department of Transportation (ODOT). If explosives are used, all Federal, State and Local regulations covering the use of explosives shall be observed. The project involves the milling and paving of Dorothy Lane from the west corporation limit to Haig Road. The work will also include curb removal and replacement, adjusting utility manholes, thermoplastic pavement markings and raised pavement markings. Pre-qualification status for the work to be performed must be in force at the time of bid submission, at the time of award, and through the life of the construction contract. Subcontractors are not subject to the prequalification requirement. The "prime" contractor must perform no less than thirty percent (30%) of the total original contract price. Each bid (proposal) must contain the full name of the party or parties making same and all persons interested therein. Proposals shall be submitted through online using the electronic forms furnished by the City and must be fully completed and electronically endorsed by the bidder. Bids must be accompanied by a bid bond equal to or exceeding FIVE PERCENT (5%) OF THE TOTAL BID AMOUNT, INCLUDING ALL ADD ALTERNATES, as a guarantee that, if the proposal is accepted, the bidder will enter into a contract as proposed.
