Reference Number 0000376460 Purchase Type: Duration: 1 year Option: Four (4) Optional Renewals The City of Lakewood (the "City") is requesting quotes from qualified individuals or entities ("Respondents") to provide Tree Removal Services on As-Needed Basis. This service may be awarded to more than one Respondent and may be extended annually not to exceed four (4) annual extensions, at the City's discretion. Award of this services under this RFQ may be non-exclusive (awarded to more than one Contractor) SCOPE OF SERVICES The successful Respondent(s) ["Contractor(s)"] will provide all necessary materials, labor and equipment for tree removals throughout the City. Work may include each of the following: 1. Tree removal a) A variety of trees and locations not all inclusive, will include parks, rights of way, medians, public and private property, and open space. b) Trees shall be removed in such a manner as to cause no damage or injury to people, property, animals, or other living plants. c) Trees shall be removed leaving a stump six (6) inches or less in height unless other arrangements are made with the City Forester representative. 2. Stump Grinding a) All contracted stump grinding shall be removed in a manner that all parts of the stump will be at least 12 (twelve) inches below the surrounding ground level unless other arrangements are made with the City Forester representative. Exposed or visible roots extending from the base of all stumps shall be removed with the stump. b) The contractor shall be responsible for all utility locates prior to any excavation. c) Wood debris from the stump grinding process shall be removed from the stump hole and backfilled with soil, up to the surrounding soil grade. 3. Traffic Control a) Coordination and implementation of approved traffic control for work completed along, or near roadways, sidewalks, and public access areas. 4. Areas of Removals a) Public parks, rights of way, medians, streetscapes, open space, and private properties. Site visits will be mandatory for each project. Contractors will meet with the City Forester or designee at each project location to provide a cost for the project.
