Contract 217 - Abandonment of Existing Sewers in Fort Monmouth. BID FOR: "Contract 217 - Abandonment of Existing Sewers in Fort Monmouth" The envelope shall plainly set forth the name and address of Bidder. The project consists of: o Maintenance and protection of traffic o Abandonment of approximately 16,500 LF of existing sanitary sewers and sanitary sewer manholes as shown and described herein. Bids shall be accompanied by a certified check, cashier's check or bid bond payable to the Two Rivers Water Reclamation Authority as a guarantee that if the Contract or Agreement is awarded to the Bidder, that the Bidder will enter into an Agreement. The amount of the guarantee shall be ten percent (10%) of the base bid but shall not exceed $20,000.00. Agreement of Surety, List of Owners, Bidder's Affidavit, Non-Collusion Affidavit, Bidders Statement of Qualifications, Public Works Contractor Registration Act Certificate, Subcontractor Listing, Equipment Certification, Acknowledgement of Addenda, State of New Jersey Business Registration Certificate, Affirmative Action, Disclosure of Investment Activities in Iran, Certification of non-Involvement in prohibited activities in Russia or Belarus, Americans with Disabilities Act of 1990 Acknowledgement, Certificate of Equal Opportunity, American Products Certification and Prevailing Wage Acknowledgment documents are also mandatory submittals. Bids shall be made on the standard Proposal Form in the manner designated therein and required by the specifications. Bids shall be enclosed in a sealed envelope, addressed to the Executive Director of the Two Rivers Water Reclamation Authority, and shall be clearly marked on the outside lower left-hand corner, "Sealed Bid - Contract 217 - Abandonment of Existing Sewers in Fort Monmouth." Bids shall bear the name and address of the Bidder on the outside. The Two Rivers Water Reclamation Authority reserves the right to waive any informality in or to reject any and all bids.
