Statement of Work (SOW) and Request for Quote (RFQ) sheet are attached. Quote must be put on RFQ and signed. The contractor shall furnish all labor, materials, equipment, tools, supervision, transportation, services, etc., required to complete the removal and replacement of one, six-inch double check valve, BPD along with all accessory fittings and adjoining valves. The BPD to be replaced is a Watts, model 774x located in a utility vault, outside of the secure confines of the institution. The contractor shall install the new BPD in existing lines and shall include removal and/or replacement of any necessary pipe, valves, flanges, fittings, etc., to perform a complete replacement. Upon completion of installation, the contractor shall perform all necessary tests to certify the newly installed BPD.
