DART is seeking a contractor to demolish, grade, and pour concrete for four (4) locations according to plans listed below: Locations to include: 1. th Avenue and Holcomb, Southbound 2. MLK Jr Pkwy and Forest Avenue, Southbound 3. University Avenue and 17th St., Eastbound 4. University Avenue and 11th St, Westbound Obtain building permit. Permits required for this construction site may include but are not limited to: Right of Way Excavation and Obstruction, Right of Way - Sidewalk and Approach Build four (4) bus bench pads according to site plans, including meeting strict and some times complex grading to Federal ADA and city sidewalk standards (noted in plans). Work Compensation: Statutory Gen. Comp. liability: $2,000,000 Auto liability: $1,000,000 All projects over $25,000 require a Performance Bond. Iowa Code573.2Must betheoriginalwith the notarized raised stamp. Iowa Code 26.8 also requires a bid bond. The bid bond will be 5%of the contractor’s bid amount and will be on a Bid Bond (preferred) or a Certified check. * *Davis Bacon does apply to this contract and the Certified Payroll form WH-347 will be required as well as Employee Wage Interviews. IWD Contractor License number is required, along with an up-to-date contractor registration.
