Permits required for this construction include Right of Way Excavation of Obstruction, Right of Way Sidewalk and Approach. Attend pre-construction meeting at the sites contingent on complexity. This project has ADA requirements.***** All Architecture and Engineering plans precede this document and must be followed. This project requires the contractor to demolish, grade, and pour concrete to specifications for two locations. Davis Bacon does apply to this contract and Certified Payroll form WH-347 will be required as well as Employee Wage Interviews. IWD Contractor License number is required, along with an up-to-date contractor registration. Work Compensation Statutory General Comp Liability $2,000,000 Auto Liability $1,000,000 All projects over $25,000 require a Performance Bond. Iowa Code 573.2 Must be the original with the notarized raised stamp. Iowa Code 26.8 also requires a bid bond. The bid bond will be 5% of the contractor's bid amount and will be on a Bid Bond (preferred) or a Certified check.
