Please contact Aaron Ketzner at (317) 222-3880 or aketzner@lochgroup.com if you have any questions or need special accommodations 2025 PARK ROAD REHABILITATION AND WIDENING PROJECT The project involves widening Park Road between 67th Street and 53rd Street, milling, wedge and level, resurfacing the pavement, and addressing stormwater management issues with ditches and storm cross pipes. Additionally, a leftturn lane will be added at the intersection of 67th Street and Park Road. The project has an intermediate completion date of November 20, 2025 and a final completion date of July 31, 2026. The plan charge will be listed on the online plan room. Payment may be made by check, credit card, or cash. NO DEPOSITS ACCEPTED. Make checks payable to Repro Graphix Inc. An online option may also be available. All payments and costs of Contract Documents and related supplemental materials are non-refundable. Bidders shall assure that they have obtained complete sets, whether hard copy or electronic, of the Project Manual and drawings and shall assume the risk of any errors or omissions in Bids prepared in reliance on incomplete sets. All bids shall be placed in a sealed envelope and identified by the name and address of the bidder and statement "2025 Park Road Rehabilitation and Widening Project". The bid shall consist of the following properly and completely executed documents: Bid shall be submitted on the enclosed "Itemized Bid 3 Form." Bid shall be submitted on "General Bid Form 96" as prescribed by the Indiana State Board of Accounts. Each bid proposal shall include a financial statement, a statement of experience, proposed plan or plans for performing the public work, and the equipment available for the performance of the public work. Each bid proposal shall include a properly executed non-collusion affidavit as required by the statues of the State of Indiana. Bid Security: Pursuant to Indiana Statute I.C. 36-1-9-6, a bidder shall file a certified check or bid bond with each bid, in an amount equal to ten percent (10%) of the total bid proposal, as evidence of good faith that the successful bidder will execute within ten (10) consecutive calendar days from acceptance of a bid proposal, a contract as set out in the specification. City of Anderson Employment Eligibility Verification. Bidder's written plan for a program to test Bidder's employees for drugs in accordance with IC 4-13-18. City of Anderson certification of Non-Segregated Facilities. City of Anderson Non-Discrimination Clause. City of Anderson Workforce Breakdown Forms. City of Anderson Affirmative Action Checklist. All Bidders must be pre-qualified for Indiana Department of Transportation work (INDOT). Failure of bidder to enter into a contract, execute same and furnish acceptable labor and materials and performance bond as hereinafter required, shall be cause for forfeiture of certified check or bid bond for liquidated damages to the City. The contractor must conform to the antidiscrimination provisions of I.C. 5-16-6, et. Seq. The performance bond and labor and material payment bond shall be for one hundred percent (100%) of the contract amount and shall accompany the executed contract. Bonds shall stay in effect for twelve (12) months after completion and acceptance of the project. Bidder will not be permitted to withdraw his bid for (60) days from the date of bid opening. The need for insurance protection against theft of or damage to materials or contractor's tools or equipment at the job site shall be the bidder's decision and expense, exclusively. The Board of Works reserves the right to waive informalities or reject any and all bids.
