SCOPE OF SERVICES TO BE OFFERED Please respond in detail as to how the Proposer will provide the services necessary to complete the project. Responses to this RFP, at a minimum, will address the following items: o a project narrative outlining how the solids will be removed from the basin at the water treatment plant. Should the City elect to do a complete versus a partial cleanout, the contractor shall be able to remove a minimum of 80% of the solids volume; o a description of the proposed equipment and processes to be used; o provisions for preparing and submitting an annual report to the Division of Waste Management along with any prior experience in doing so; o a cost for each aspect of the operation (cost sheet has been provided); o describe the sampling and analysis program to ensure regulatory compliance with the Beneficial Reuse permit; o insurance coverages; o proposer's work experience, history and three references of similar projects in Kentucky; o information regarding any subcontractors proposed to be utilized and; o any other details useful to the City in order to select the best proposer for the project. The City of Ashland, KY (City) is seeking the services of a qualified and experienced firm to remove and dewater solids from its 25 million gallon reservoir. The City desires proposals for a turnkey operation to remove, dewater and transport solids from the raw water storage basin at the water treatment plant.
