Renovation of a fire / police facility in Toledo, Ohio. Conceptual plans call for the renovation of a fire / police facility.

The City of Toledo DPS/FAFO is seeking proposals for the complete design for building renovations and improvements for our Police operations. We are looking at acquiring a building located at 211 S. Byrne rd. Toledo Ohio. Other Details Proposals for this project can only be accepted via electronic submittal through the City of Toledo's online website. Bid submittals or other transmissions of information have not been completed until an email confirmation of the transmission has been issued by online. Bids prices shall be to two (2) decimal places (i.e. $0.00). Only one (1) price per line item will be accepted with this bid. In order to be responsive, all proposals must have the following uploaded with submittal in online: o Letter of Transmittal o Project Paperwork* -Acknowledgement and Acceptance of Conditions and Contract Specifications -EEO Form -Good Faith Effort Form -Living Wage Affidavit/Living Wage Contract Addendum -City of Toledo Public Utilities Compliance -Toledo Income Tax Compliance -Primary/Subcontractor Agreement Certificate -Debarment and Suspension Certificate o Child Support Affidavit* o City of Toledo Professional Services Agreement* o Insurance Certificate, listing the City of Toledo additionally insured o Bureau of Workers Compensation Certificate o Cost proposal City forms must be filled out and attached to the bid. Project Paperwork includes the following: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate; Best Bid Criteria.
