The Waterbury Engineering Department is looking for a site/paving contractor to reclaim and pave a portion of the parking lot at St Patrick's Church, 50 Charles Street, Waterbury, CT, remove existing and then provide new bituminous concrete lip curb, pavement markings and incidental related work. The balance of the parking lot will be rehabilitated under a separate contract All work and materials shall be in accordance with the Connecticut Department of Transportation, Form 819 for Road, Bridges, Facilities and Incidental Construction, as amended and supplemented. Scope of Work: Reclaim approximately 670 square yards of existing bituminous concrete to a depth of approximately six inches (6"), remove approximately 210 linear feet of existing bituminous concrete lip curb (BCLC), fine grade and compact, furnish and install 1.5 inches compacted of bituminous concrete binder course, 1.5 inches of compacted bituminous concrete surface course, furnish and install approximately 210 linear feet of bituminous concrete lip curb, furnish and install loam, seed and mulch compacted to top of curbing, and furnish and install 4" wide white epoxy pavement markings for parking stalls. Certificates of Insurance: The Contractor's General, Automobile, Builder's Risk and Excess/Umbrella Liability Insurance policies shall be endorsed to add the City and its Board of Education as an additional insured and provide waiver of subrogation on all policies except Builder's Risk and Pollution Liability. The insurance afforded the additional insured shall be primary and non-contributory insurance and the coverage and limits provided under the Contractor's policies shall not be reduced or prorated by the existence of any other insurance applicable to any loss the additional insured may have suffered. Prior to the execution of this Contract by the City, the Contractor shall furnish to the City, subject to City approval, certificate(s) of insurance and Additional Insured Endorsement and Waiver of Subrogation Endorsement verifying the above coverages, including the naming of the City of Waterbury, as follows: "The City of Waterbury and its Board of Education are listed as additional insured on all lines of coverage on a primary and non-contributory basis on all policies except Workers Compensation and Professional Liability. All policies shall include a Waiver of Subrogation". The City's Invitation to Bid Number must be shown on the certificate of insurance to assure correct filing. The Contractor must supply replacement/renewal certificates at least 30 days prior to the expiration of the policy(ies). Said certificates shall contain a provision that coverage afforded under the policies shall not be cancelled or reduced for any reasons unless notice of than thirty (30) calendar days has been mailed to the City's Using Agency and a copy to the City's Office of Corporation Counsel, 235 Grand Street, Waterbury, CT 06702. No later than thirty (30) calendar days after receipt by the Contractor, the Contractor shall deliver to the City a copy of the Contractor's insurance policies and endorsements and riders.
