Multi-Door Replacement & Repair Amendment 1: Change to the Site Visit: RFP page 57, section L.5: The RFP initially stated "(b) An organized site visit is not scheduled; if interested prospective offerors can drive by and take a look at the site prior to preproposal conference. o If needed, please coordinate a site visit with the Contracting Officer and COR." The site visit information is amended to read: "(b) An organized site visit is scheduled for 11:00 AM Central Time. Interested offerors who want a site visit will meet the IHS representative at the South Hospital Entrance." Acknowledge all solicitation amendments in Block 19 of the SF1442. End of Amendment 1. The work includes the replacement and repair of multiple doors at the designated facility. The contractor shall provide all labor, materials, equipment, and supervision necessary to perform the required door replacements and repairs in accordance with the specifications and standards outlined in the solicitation. The scope encompasses ensuring proper functionality, safety, and compliance with applicable building codes for all affected doors. Period of Performance: 180 days. Questions should be received no later than 7 days prior to the proposal receipt date to allow time for a response. The contractor shall begin performance within 10 calendar days and complete it within 180 calendar days. Description: The project defined within this Scope of Work is for the replacement of 15 doors and repair of 4 doors throughout the Quentin N Burdick Memorial Healthcare Facility. E. Background: Doors throughout the Quentin N Burdick Memorial Health Care Facility are damaged, worn, and not functioning properly. This project will correct these issues through replacement and repair of a total of 19 doors. C-2. Scope of Work. (SOW) The scope of this project includes all demo, new installation, and clean up for the replacement of the 15 doors and repair of the 4 doors indicated below and in the attached plan sheets. The contractor shall be responsible for all necessary patching repair and painting resulting from the removal and installation of the new doors and frames. This includes, but not limited to, repairing adjacent wall surfaces, trim, and finishes as needed to match existing conditions. All disturbed surfaces shall be finished to match surrounding material in color, texture, and quality. The contractor shall provide all engineering, design, material, labor, equipment and supervision required to accomplish the following: A. Phasing: The contractor shall coordinate with the project engineer to phase door installation to minimize disruption to facility operations. The government reserves the right to limit the number of doors being worked on simultaneously based on operational needs, site conditions, or other considerations. The contractor shall perform the work in a manner that allows the facility to remain functional and secure throughout the duration of the project. B. Materials: All materials shall be stored and handled according the manufacturers' recommendations. All materials shall be submitted to the COR prior to ordering, for approval by the project engineer. Contractor shall not begin installation until all materials required to complete installation have arrived on site. C. Temporary Construction Barriers: The contractor shall provide temporary partitions to separate work areas from owner-occupied areas, to prevent penetration of dust and moisture into owner occupied areas, and to prevent damage to existing materials and equipment. The contractor shall utilize a hepa filtered air scrubber within the temporary barrier. D. Demolition: The contractor shall remove the doors & frame as required in attached design sheets. Demolish existing work as required to accomplish installations indicated on attached plan sheets. The removal of waste and debris is the sole responsibility of the contractor. Waste shall be disposed of in an approved landfill. E. Installation: The contractor shall conduct an on-site verification of all existing conditions, dimensions, and other relevant project details. The contractor is responsible for ensuring all measurements, materials, and installation requirements align with the actual site conditions prior to ordering materials or beginning construction. a. Automatic Sliding Doors (Basis of Design): i. ii. (TSLD) Stanley Dura-Fit: The contractor shall furnish and install four (4) Stanley Dura-Fit series Bi-Part sliding doors or approved equal at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer's specifications and project requirements. Door finish shall be clear anodized aluminum. All doors shall include: full breakout, 10" bottom rails, access control, and 1" insulated glazing. Doors to include, where relevant, transoms and side lites to match existing opening. Transom and side lite glazing thickness shall match door. Hardware per manufacturer's specifications. (BSLD) Stanley Dura-Glide: The contractor shall furnish and install one (1) Stanley Dura Glide 3000 series Bi-Part sliding doors or approved equal at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer's specifications and project requirements. Door finish shall be clear anodized aluminum. All doors shall include: full breakout, 10" bottom rails, access control. Interior door(s) shall include 1/4" glazing. Exterior door(s) shall include 1" insulated glazing. Doors to include, where relevant, transoms and side lites to match existing opening. Transom and side lite glazing thickness shall match door. Hardware per manufacturer's specifications. b. Aluminum Framed Storefront Doors: The contractor shall furnish and install two (2) aluminum store front doors at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer's specifications and project requirements. Door finish shall be clear anodized aluminum. Interior door(s) shall include _" glazing. Exterior door(s) shall include 1" insulated glazing. Doors to include, where relevant, transoms and side lites to match the existing opening. Transom and side lite glazing thickness shall match door. See hardware sets below. c. Hollow Metal Doors: The contractor shall furnish and install two (2) hollow metal doors at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer's specifications and project requirements. Door shall be factory primed and painted. Contractor shall submit from manufacturer's range of colors for approval. Doors to include, where relevant, transoms and site lites to match existing opening. See hardware set below. i. ii. C027b: Door level shall be Level 3 - Extra Heavy Duty. Based on SDI standards: ANSI/SDI A250.8 (SDI-100). Door R value shall be minimum R-11. B189a: Door level shall be Level 2 - Heavy Duty. Based on SDI standards: ANSI/SDI A250.8 (SDI-108). d. Solid Core Wood Doors Non-Fire Rated: The contractor shall furnish and install five (5) solid core non-fire rated 20-min wood doors at the locations designated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer's specifications and project requirements. The door finish shall be wood veneer, submit from manufacturer's full range of finishes for approval. e. Solid Core Wood Doors 45-Min Fire Rated: The contractor shall furnish and install one (1) 45 min fire rated wood doors at the locations indicated in the attached plan sheets. The doors shall be installed in accordance with the manufacturer's specifications and project requirements. The door finish shall be wood veneer, submit from manufacturer's full range of finishes for approval. The doors shall be installed to comply with NFPA 80 & NFPA 101. f. Hollow Metal Frames: All non-aluminum doors shall have full profile/continuously welded hollow metal frames. Frames shall be factory primed and finished. Frame gauge shall be 16 gauge minimum. Fire rated hollow metal frames shall be labeled according to NFPA 80. See attached design door replacement schedule on sheet A.8 indicating where frames are required. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
