This work includes, but is not limited to, the installation of a 36-inch butterfly valve at the intersection of Columbia Avenue and Summer Street. Work also includes the installation of two 36-inch line stops on existing line stop connections (Re-Stop Line) including excavation and restoration of the asphalt. All work is located in the City of Hammond, Lake County, Indiana. Bids must be properly and completely executed on the forms contained in the Contract Documents. All bidders must be properly licensed in the City of Hammond at the time bids are received. Bids must be accompanied by a certified or cashier's check or Bid Bond of no less than 5% of the bid price. Performance and payment bonds, each in the amount of 100% of the contract price, will be required. The Water Works Department reserves the right to waive informalities and to award the contract to any bidder to the advantage of the Department, or to reject all bids. Contractors must subscribe to the Indiana Plan and a joint apprenticeship and training program if their bid is accepted. Each bidder must submit an Affirmative Action Statement at the time of bid and ensure nondiscrimination in employment. Standard Indiana Form 96 (Revised 2013), including a non-collusion affidavit, is required. Bids shall remain in effect for 60 days following the bid deadline. Additional instructions pertain to licensing requirements and the acquisition of bid documents from NIES Engineering for a non-refundable fee in electronic PDF format. Pre-qualification requirements: All bidders must comply with Hammond Ordinance No. 9519 regarding Responsible Bidding Practices and must become prequalified by the City of Hammond Building Commissioner prior to submitting a bid for public works. All bidders must be properly licensed in the City of Hammond at the time bids are received. Proof of subscription to the Indiana Plan and appropriate apprenticeship and training programs is required before payment can be made. An Affirmative Action Statement must be submitted at the time of bid. Bidders must not discriminate against any employee or applicant for employment based on race, creed, color, sex, or national origin and must comply with anti-discrimination provisions of IC 5-16-6.
