Renovation of a medical facility in Tupper Lake, New York. Completed plans call for the renovation of a one-story above grade medical facility.
The Vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work. Work is to be completed within 30 days of receiving Authorized Purchase Order, unless modified below. This is a NYS Public Works project, and prevailing wages are to be paid, and Certified Payrolls must be submitted at time of completion. Site must be clean, and all materials secured at the end of each workday. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of work, but before removal of equipment, contractor is to schedule a quality control and quality assurance inspection with point of contact and project supervisor Max Gerstenberger. OPWDD is soliciting quotes from responsible and qualified MWBE, SDVOB, or NYS small business contractors to provide asbestos abatement service to Sunmount DDSO, Building 4 flooring. This procurement opportunity is limited to New York State small businesses, New York State businesses certified pursuant to Articles 15-A and 17-B of the New York State Executive Law--Empire State Development Corporation Certified Minority or Woman Owned Business (MWBE) or New York State Office of General Services Service-Disabled Veteran-Owned Business (SDVOB) only. Please provide a copy of your firm's certification documents when submitting a quote. Mandatory site visit required. Contract term is one year. Location is Sunmount DDSO 2445 State Route 30 Tupper Lake, NY 12986.
