The city of Toledo encourages all bidders to achieve a 15% MBE/WBE participation on all construction contracts. This project will resurface 0.86 miles of pavement on East Broadway Street, from Oakdale Avenue to Woodville Road. This includes removing the existing wearing course, performing necessary pavement repairs, resurfacing, the installation of pedestrian ramps, and minor drainage improvements. Bids for this project will be accepted via e-bid through the City of Toledo's PlanetBids portal. Bid submittals or other transmissions of information are not complete until confirmation of the transmission has been issued by PlanetBids. Bid prices shall be to two decimal places. Only one price per line item will be accepted. In order to be responsive, all bids must have the required documents uploaded, including: Construction Project Paperwork, Line Items, Bid Guaranty (bid bond or certified check/cashier's check on a solvent bank), Signed Contract by the Vendor, Child Support Affidavit, Bureau of Worker's Compensation Certificate, Tax Exemption Certificate, Insurance Certificate naming the City of Toledo as an additional insurer. Bid Guaranty requirements are located on the Bid Information tab on PlanetBids, and the bid guaranty shall be in the form of a cashier's check/official check on a solvent bank or a bid bond. Company and personal checks will not be accepted. If a bid bond is used, scan and upload it with the bid; the original does not need to be received by the City. City forms must be filled out and attached, including: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate; Best Bid Criteria. Other required forms include those noted in the Response Type Section and the Other Details Section: Signed Contract by Vendor CEO/President, Child Support Affidavit, Insurance Certificate listing the city as additional insurer, and Current Worker's Compensation Certificate. Upon award, a Performance Bond and PLA, if applicable, will be required. The successful bidder must submit certified payrolls to the City for all labor performed on the project, two weeks after start date then weekly thereafter. Prevailing wages are required for this project.
