The bid proposal must include the vendor's Universal Service Fund (USF) Service Provider Identification Number (SPIN) current for the 2026 Funding Year. The project may be eligible for Year 29 USF funding, and no invoices will be dated or paid before July 1, 2026. The contract is 100% conditional upon Universal Service Funding, and the District reserves the right to reduce the contract amount or scope to meet budget requirements if the USF discount is reduced. Unit pricing, as specified in the bid proposal, will be used to control contract adjustments. The District reserves the right to specify the reimbursement filing option for implementing E-rate discounts (BEAR or SPI). The E-Rate funding Excel Spreadsheet must be submitted with the original bid response, including manufacturer, part number, quantity, and unit pricing for each component, and bidders must indicate the percentage of E-rate eligibility for each cost. A single lump sum bid proposal is required for the described work. Each bid proposal must include: - A sworn and notarized statement disclosing any familial relationship between the bidder (or employees) and any member of the Owner's School Board or Superintendent. - A sworn and notarized Iran Sanctions Affidavit. - A sworn and notarized Affidavit for Criminal Background Checks. - Bid security in the form of a certified check, cashier's check, or satisfactory surety bid bond in an amount not less than five percent (5%) of the total bid price.
