Remodeling of a municipal facility in West Milwaukee, Wisconsin. Completed plans call for the remodel of a municipal facility.

Scope of Work Item Code Estimate EXTERIOR HOUSE 1 Roof Replacement: Tear off all layers of existing shingles and replace the entire roof. Replace any underlying rotten or damaged roof sheathing as needed. Install 'W' flashing at all valleys. Provide ice and water shield a minimum of 3 feet wide at all roof edges, and valleys, install synthetic roofing felts, and metal drip and apron flashing. Install new roof vents per code. Provide all necessary flashings, and minimum 30 year dimensional shingles. PR 2 Roof Warranty: Shingle manufacturer's warranty to be provided to the homeowner prior to payment for roof replacement. PR NA 3 Fascia and Exterior Trim Painting: Scrape, prime paint, and finish paint the fascia on the front right corner and the crown molding on the bumpout on the south side of the home. Colors per owner. CV, Pb 4 Gutters and Downspouts: Replace all gutters and downspouts. (Note: For gutters and downspouts from upper dormers, chase all discharge through downspouts to lower gutters; do not discharge water across lower roof surfaces.) Where discharge to grade is problematic, connect downspouts to existing storm inlets with sealed boots. For other downspouts, direct discharge to grade with bends and extensions 4' away from the foundation, and provide splash blocks. Install permanent caps at abandoned storm inlets. Install new downspout at the NW corner. CV, Pb 5 Chimney Repairs: Reflash the chimney with metal flashing color matched to the roof. Work to be conducted in conjunction with the roof replacement. When installing new flashing at the chimney, the flashing must be stepped or cut into the mortar joints. The bricks may not be cut to install flashing at an angle. CV Miscellaneous: The contractor is responsible to field verify all measurements, the amount of materials needed, and the number of windows in the building. If any item in this scope calls for a certain manufacturer, model number, or approved equivalent of a particular item, and that item is to be substituted, both NIDC and the owner must approve the substitution in writing as a part of the contract. Both the "Rehabilitation and Technical Specifications and Performance Standard for the City of Milwaukee - February, 2006" and the "NIDC Addendum to Specifications ," dated 8/23/07 & 5/13/08, are incorporated into this scope by reference. These items provide an outline of materials requirements & performance expectations. Updated copies can be obtained from NIDC @ 809 N Broadway-3rd Flr. Lead Related Work: Any task that a disturbs previously painted surface shall be performed by properly certified personnel and follow the State of Wisconsin Department of Health Services Administrative Code Chapter DHS 163 regarding the certification for the identification, removal and reduction of lead-based paint hazards (Pb). And, if applicable: Any task that disturbs a previously painted surface requires Milwaukee Health Department notification (Pb-N). Window replacement shall be performed by properly certified personnel and requires a Milwaukee Health Department Permit (Pb-A). YES NO 6 Front Entry Porch (concrete): Repair the failing concrete on the underside of the front porch flooring. This area Is accessible from the basement and will need to be formed overhead and the sagging rebar will need to be re-emedded into the new concrete. CV 7 Brick Tuckpointing - Mechanically remove the failed mortar on the west side of the north bumpout and both sides of the lower portion of the chimney along the side of the home. Re-point those areas. New mortar to be "N" type mortar and color matched to the existing. CV 8 Rear Jump Porch: Scrape, prep, prime and pain the wooden posts on the rear jump porch. Color and sheen per owner. Replace the existing metal handrail with either a black metal or pressure treated code compliant hand rail. If metal the guardrails are to be Designer's Image (Menards) aluminum posts and rails, powder coated in black. CV, Pb 9 Broken Glass Pane: Replace broken glass piece in the casement window on the rear NW corner of the home. If the glass pane cannot be repaired then replace the entire sash with a like replacement. CV, Pb INTERIOR 1ST FLOOR 10 Alarms: Install a combination smoke/CO alarm in the basement, first floor, second floor. All alarms shall be sealed battery, 10 year lithium ion units. CV SELF-HELP 11 GFCI Receptacles: Replace the existing electrical receptacles at wet areas with ground fault protected receptacles at the following locations; one by the sink in the 1st floor kitchen. PERMIT, CV 12 Rear Stairwell Structural Repairs: Repair the sagging landings in the rear stair case. Repair or rebuild wall and stair framing as needed to bring the landings level. Replace any treads, risers or other stair components after framing repair is complete. Any drywall or plaster that was removed during the repair process to be reinstalled, repaired and repainted to match their existing texture and finish. PERMIT, CV, Pb 13 Push Button Switches: Replace the 4-gang push button light switches in the dining room with a modern code complaint light switches. PERMIT, CV 14 Shower Light: Install a light rated for wet enviroments in the shower stall in the 1st floor bathroom. If possible tie this new fixture into the 2-gang box on the outside of the shower PERMIT, PR 15 Bathroom Windows: Install a new waterproof layer over the interior of the existing casement windows. This could be a waterproof sheeting or glass block. Remove any wooden trim on the interior. The exterior sashes must remain per HPC guidelines PR 14 Bathroom Fan: Reattach or replace the loose bath fan in the 1st floor bathroom. CV, PERMIT 15 Tile Floor: Replace the damaged tiles around the base of the radiator in the 1st floor bathroom. New tiles and grout to a close match to the existing. New tiles to be attached to a subfloor or substrate rated for tile install. CV 2ND FLOOR 16 Push Button Switches: Replace the push button light switch in the rear stairwell with a modern code complaint light switch. PERMIT, CV 17 Rear Stairwell Lights: Replace the damaged rear stairwell light. Design per owner. Inspect the light by the 1st floor kicthen door to see if there is a corresponding light switch. If not then install a light with a pull chain or built in switch. PERMIT, CV 18 GFCI Receptacles: Replace the existing electrical receptacles at wet areas with ground fault protected receptacles at the following locations; one by the sink in the 2nd floor bathroom. PERMIT, CV BASEMENT 19 Electrical Cover Plates: Install outlet, switch, and junction box cover plates at all locations where missing. Noted areas: basement in the boiler/mechanical room. CV 20 Electrical Panel: Replace the missing breaker or install a blank cover in the panel for the lower unit PERMIT, CV 21 Pull String Light: Replace or reattach the pull string on the light in the rear mechanical room. PR 22 Lifting of the Central Beam: Install permanent jacks for the main (5) posts holding up the central beam. Lifting of the central beam to be no greater than 1/8 per day and to not exceed 1 inch or per the engineering report. Any repairs to the existing footers would need to be at the recommendation of a structural engineer. Prior to any work on the central beam or the pillars the contractor must obtain an engineering report. PR 23 Drywall Removal: Remove areas of moldy or damaged drywall in the basement kitchen, bathroom and SW bedroom. Confirm locations with the Rehab Specialist. Reinstall new drywall and tape, float, prime and finish paint the repaired areas. CV, Pb 24 Boiler Inspection: Inspect the existing boiler for proper function. Ensure all gas lines are correctly attached and sealed CV 25 Cracking and Seperation Post Beam Lift - Repair and patch all cracks in the walls and ceilings that have resulted from the lifting of the central beam. Prime and paint all of these areas to match their existing finishing and texture. Please contact Davis Jones at (414) 286-6458 for new Rehab Specifications. The Owner reserves the right to reject any and all bids and to waive informalities. It is NIDC policy that, if: (a) the Contractor's proposal/bid complies with the specifications and other requirements, and (b) the Contractor meets NIDC's minimum requirements as outlined herein, and (c) the selected bid is reasonably within range of the market price for the work, then Owner may select any Contractor who, in Owner's opinion, is the most responsible and responsive bidder *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
