The project is for the 2025 Asphalt Seal and Preventative Maintenance Project for the City of Anderson, Indiana. Bids must be in accordance with bid plans and specifications available through the City Engineer's Office. The City reserves the right to delete any section(s) of the bid proposal, and all materials, supplies, and work to be furnished or performed must comply with requirements set by the Board of Public Works. Instructions to bidders: Bids must be placed in a sealed envelope, identified by the name and address of the bidder and labeled as the "2025 Asphalt Seal and Preventative Maintenance Project for the City of Anderson, Indiana." Bids must include the following documents, properly and completely executed: the "Itemized Bid Form," "General Bid Form 96" as prescribed by the Indiana State Board of Accounts, a financial statement, a statement of experience, proposed plan(s) for performing the work, information on available equipment, and a non-collusion affidavit. Each bid must include a certified check or bid bond for 10% of the total bid, as evidence of good faith. Additional required documents include City of Anderson Employment Eligibility Verification, a written drug-testing program, certification of Non-Segregated Facilities, Non-Discrimination Clause, Workforce Breakdown Forms, and Affirmative Action Checklist. Failure to enter into a contract and provide the necessary bonds may result in forfeiture of the bid security. The successful bidder must provide performance and payment bonds for 100% of the contract amount, effective for twelve months post-completion. Bids cannot be withdrawn for 60 days from the opening date. Insurance against theft or damage to materials or equipment is the contractor's responsibility and expense. The Board of Works reserves the right to waive informalities or reject any and all bids.
