The work includes providing all labor, materials, and equipment, and performing all necessary and incidental work in accordance with the Statement of Work. Submissions must include the Quote Sheet, required forms, proof of contractor's license, EPA certification, and insurance coverage with a company cover letter, to be uploaded as instructed. All quotes must be labeled accordingly and submitted following the provided procedures. Contractors must agree to execute a standard LACDA contract if awarded. Minimum requirements include a current and valid Class "B" or "B-2" contractor license, EPA "Lead-Safe" Certification, insurance coverage (general liability, workers' comp, auto, crime, pollution/asbestos/errors & omissions), and a contractor license surety bond. Work completion is required within 30 calendar days from the Notice to Proceed, with penalties for non-completion. Additional requirements include compliance with child support, administration forms, lobbyist ordinances if applicable, property tax reduction, EEO, fair chance hiring, GAIN/GROW, jury service, safely surrendered baby law, and zero tolerance on human trafficking policies. Contractor shall fully and timely perform all work required to rehabilitate and improve the Property, the scope of which is more fully described in the "Notice of Request for Quotes (SGD 15-158) for Senior Grant Program" and the "Senior Grant Program Check List and Work Scope" attached hereto as Exhibit 2 (the "Work"). The Work shall include all acts reasonably necessary to complete the Work including, but not limited to, furnishing all personnel, labor, materials, machinery, tools, equipment, fixtures, transportation and any other services reasonably related to the Work (including, but not limited to, supervision of workers), whether or not expressly set forth in the Scope of Work.
